Nut falling off Gooseneck / unsure what to do

Anyone familiar with the arrangement of washers on the gooseneck? Perhaps I set this up wrong.

The problem I am encountering is that the nut falls off, and it being warn down now, I found it impossible to find a replacement the same size, instead using one slightly taller and that is falling off as well if for no other reason there are not enough threads to reach the nylon locking part of the nut.

Anyone solve this problem?
Any pictures of what washers belong where? I have a mix of synthetic and metal washers I may not have properly allocated.

Just so happen to be on the boat and saw your post. There should be a cotter pin through the bottom of the bolt to prevent the nut from falling off. As far as washers, mine has one at every space except where one would think it's most important, between the bottom of the boom and gooseneck. Not sure how important that is, seems to be working fine for at least the last couple years now. Or maybe that would have been the difference between a mid pack finish and first! :P. Sorry, can't figure out how to rotate the pictures in the post (they are upright as a file on my computer).

Did the goose neck come manufactured with the hole in which the cotter pin is inserted, or did you drill that somehow without damaging the threads?

All of the ones I've seen have the hole for the cotter pin.

David Lively from Sparcraft says the replacement one's available don't have a hole. I wonder if I can drill a hole without damaging threads?
