Thru hull fitting issues

Has anyone else dealt with thru hull fittings separating right at the hull outer skin level? Boat is 2003 and just had it surveyed. Moisture meter only responded at the edge where there was visible water, but want to sort this before any water has a chance of getting in. Looks like blocks that thru hull fittings are mounted to are solid.

Just wondering if I could scrap the plastic trim piece, build up the edge with high density west system to make it flush, seal it with Interprotect, and re-paint or if I need to go deeper.
